Chronic Disease Management Plans
A Chronic Disease Management Plan (CDM) is a GP referral to an Allied Health professional where up to 5 appointments receive a Medicare subsidy.
A CDM can be put together by your GP if you:
It is up to the GP to determine whether you are eligible for a CDM and how many Physiotherapy and/or Osteopathy appointments you receive (maximum of 5 per 12 months). Please consult with your GP for more information.
Those who are referred to us with a CDM pay the full consultation fee at the time of the appointment. Spark Health Co will process the Medicare claim and the rebate will be placed directly into the patient’s bank account linked with Medicare. We must have received the paperwork from your GP before we can commence the plan.
You can choose to continue treatment after your CDM appointments are completed, and use other forms of cover if applicable e.g. private health insurance.
