Physiotherapy Home Assessments
Our Physiotherapists can conduct an in-depth assessment of your home environment, how you move within it and look at safety issues and possible falls risks. We can recommend strategies to make home life easier and safer for you, including the use of aids and equipment and some adjustments in the set-up of your home to ensure you can be safe, functional and independent.
Physiotherapy Home Assessments are available for:
– NDIS participants
– Support at Home (SaH) clients


What does a Home Assessment involve?
In-depth walk through of the home, in particular the entries and exits, living areas, kitchen, bedrooms and bathrooms to see for any safety concerns and falls risks
Functional assessment with the client of how they: get in and out of the bed, shower and chairs; get on and off the toilet; dress themselves; mobilise around the house; and prepare and consume food & drink
Written report and recommendations for any appropriate changes to be made within the home, and any equipment to purchase, that will increase safety, function and independence within the home
Home Assessments are available to new and existing clients in Numurkah, Nathalia, Shepparton, Cobram and Yarrawonga areas.
We have a strong affiliation with a leading supplier of Assistive Technology and mobility aids and are willing to provide quotes and purchase any necessary equipment to make the process simple and streamlined.
How to organise a Home Assessment?
Complete the Home Assessment Application Form
Form can be completed by either the client or their Plan Manager/Support Coordinator
Await a Call or Email to book the Home Assessment
We endeavour to complete the Home Assessment within 2-4 weeks from application submission
Ensure a Safe, Functional and Independent Home
